摘要
表皮生长因子(EGFR)络氨酸激酶抑制剂(TKIs)改变了对于非小细胞肺癌(NSCLC)的治疗模式。因为表皮生长因子受体激活突变的存在,导致了表皮生长因子受体的分子生物学研究在临床试验中选择病人需要更加准确。尽管如此,一些缺乏EGFR-TKIs响应或者只具有短暂响应的患者也被发现了。这敦促了潜在耐药机制的研究。最重要的是表皮生长因子受体二次突变的存在,例如T790M,或者间叶细胞上皮细胞转变因子(MET)也许可以解释为什么病人最开始对EGFR TKIs响应,而最终却不再响应。已经采取了许多方法,及针对EGFR耐药变异的新药或者MET目前也正在开发。这里我们对表皮生长因子受体生物途径和目前在市场上已经获得批准的EGFR TKIs的临床数据进行了综述和更新。一些正在研究的针对耐药变异或者针对EGFR和其他相关受体的新化合物也进行了综述和讨论。

摘要: Epidermal Growth Factor Receptor (EGFR) tyrosine kinase inhibitors (TKIs) have changed the paradigm of treatment in non-small cell lung cancer (NSCLC). The molecular biology study of EGFR has led to clinical trials that select patients more accurately, regarding the presence of EGFR activating mutations. Nonetheless, a lack of response or a temporary condition of the response has been detected in patients on EGFR TKIs. This has urged to study potential resistance mechanisms underneath. The most important ones are the presence of secondary mutations in EGFR, such as T790M, or the overexpression of mesenchymal-epithelial transition factor (MET) that may explain why patients who initially respond to EGFR TKIs, may ultimately become refractory. Several approaches have been taken and new drugs both targeting EGFR resistance-mutation or MET are currently being developed. Here we review and update the EGFR biological pathway as well as the clinical data leading to approval of the EGFR TKIs currently in the market. New compounds under investigation targeting resistance mutations or dually targeting EGFR and other relevant receptors are also reviewed and discussed.
